Examination Forms

Syllabus notes can be requested by contacting the CDTA office.

Please note that all of the forms have been approved by the Ontario Branch Executive. Each division may have some slight variations according to their particular needs, but we have attempted to standardize these forms as much as possible.

Member teachers may contact the office via email or telephone to receive current "form B" rate tables. The office administrator is available at 905-564-2139 Tuesday through Friday from 10 am to 3pm.
